Cryoprocedure

Cryoprocedure:

Remember that you must complete SNOT 22 before the procedure!!
Ask if the patient has taken pre-medication (Panodil 1 g + Ipren 400 mg)

Preparation:

Xylocaine-adrenaline/ephrine spray in both sides of the nose

Rhinoscopy bilat with 0 gr rigid optics

In case of greater septal deviation/spina formation, the cryoprocedure cannot be carried out, possibly it can be carried out unilaterally (same price), ask the patient.

Detumescence with tetracaine phenyl ephrine 2 ml on cotton wool/ nasal tampon in each side of the nose for 10 min. If possible, remember to get the swab/nasal tampon completely posterior to the posterior edge of the conchae inferior.

Procedure:

The patient is placed in a supine position with the head elevated 45 degrees.

Cotton wool/nasal tampon is removed

The cryoprobe is placed in 2-3 places on the conchae inferior (depending on space conditions), first along the lower edge, then along the medial surface and, if possible, along the upper edge. Most patients only have room for freezing 2 places on each side of the nose. Every place is frozen for 45 seconds by activating the pedal/helper. It is important to wait 5 seconds until the probe is no longer frozen to the mucosa before moving it. In addition, it is best kept free from the septal mucosa.

ATTENTION For the indication runny nose, it is important to reach and freeze especially on the back part of the conchae inferior/just superior for this sv.t. the lower along the nerve plexus (Ganglion Pterygopalatinum)

After the procedure:

The nasal cavity is carefully cleaned with suction.

Saline spray and Nasal steroid spray are provided, the patient is instructed in their use and the doctor draws the patient’s attention to the user instructions in the “Patient information”.

The patient is informed that there will be a lot of secretions from the nose in the following days and that they may blow their nose, but not too forcefully as this could result in bleeding.
Remember to tell the patient that their nose will be completely blocked, but that the swelling in the nasal lining will subside over a few days.

Full effect of the treatment after 2-6 weeks.

Information regarding nasal spray:

Saline nasal spray 1-2 puffs in each side of the nose morning and evening for 1-2 weeks or as long as the patient feels stopped.

Ryaltris 1-2 puffs in each side of the nose morning and evening for 1-2 weeks or as long as the nose feels blocked.

In case of continued discomfort with dryness/crusting in the nose, the patient can buy Nasal Spray with Sesame Oil at the Pharmacy (instructions on the package).

Remember to give times for:

Cleaning of the nasal cavity with suction by a doctor after 7-10 days.

Medical check-up after 6 weeks including completion of SNOT 22
